Providing Critical Care for Underserved Patients in Western Michigan
Those affected by homelessness make up an incredibly vulnerable part of the population. Without consistent access to basic needs (like shelter and food), people experiencing homelessness along with chronic illnesses are often not able to receive medical care they desperately need.
To address this, BHSH Spectrum Health West Michigan, a not-for-profit division of the BHSH system, worked with community partners to launch a pilot program that aimed to help chronic illness sufferers impacted by homelessness get the routine care they need. Through this workflow using Lightbeam’s Cohort Builder, a community health worker was able to identify a diabetic man affected by homelessness and helped forge positive environmental changes that made the patient’s condition easier to manage.
