Embracing Health Equity & Scaling Population Health: Lessons from Esse & UnityPoint
Providers across the U.S. face the dual burden of overcoming staffing shortages while extending care management to larger value-based care populations with more complex needs. Population health leaders are turning to technology to engage vulnerable and rising-risk patients and proactively pinpoint the 1-2% that need support. Better data is empowering staff to work at the top of their clinical licenses and support chronic conditions, as well as social determinants of health (SDoH) in the pursuit toward health equity. Leaders from Esse Health and UnityPoint Health will share the population health strategies they have championed to prevent avoidable utilization, improve health equity, and scale care delivery.
Learning Objectives:
- Explain the technological and staffing barriers to scaling population health initiatives and strategies to overcome them
- Describe how AI models include non-clinical factors and the impact on predicting utilization and patient outreach precision
- Identify the ways in which real-time alert data promotes health equity by enabling top-of-license care management
